Goodwill Industries of Greater Cleveland and East Central Ohio is currently seeking a full time Case Manager for our Good Partner Program who will assume primary responsibility for providing case management services, social services navigation, coaching, and other supportive services to employees of Goodwill Industries of Greater Cleveland and East Central Ohio, Inc.

The Good Partner Case Manager provides support and resources to individuals in need of support to address barriers to employment, including transportation, housing, access to healthcare, and other social, cultural, and environmental conditions that impact the individual's life. The Case Manager will help to identify areas of need and work with the individual to identify goals and develop plans to address these barriers in an effort to allow the individual to maintain employment.

Our Case Manager will:

  1. Conduct initial intake and assessment with GIGECO staff to identify areas of need and develop appropriate program plans to address needs.
  2.  Develop, in conjunction with the employee an Individualized Program Plan (IPP) that identifies areas of need, sets goals, and outlines a plan for  achievement of goals.
  3.  Provide individualized case management services to GIGECO staff based on individual needs and program plans. Case management services may include information and referral to necessary community supports.
  4.  Provide skill development and job coaching services where necessary based on the needs of the individual served.
  5.  Clearly records individual progress through services and complete all necessary documentation and reporting within established timeframes.
  6.  Complyies with applicable CARF standards, federal, state, and local laws.
  7.  Complyies with agency policies, procedures, contractual guidelines, safety and security regulations, and rules protecting participant confidentiality.
  8.  Conduct employee outreach efforts in order to explain the available services.
  9.  Attends all internal and external meetings and trainings as required.
  10.  Completes other duties as assigned.

Specific Experience/Skills/Abilities:

  • Experience in the social work or human services field.
  • Extensive knowledge of available community resources and supports.
  • Experience working with persons with diverse backgrounds, individuals with disabilities, or individuals with other barriers to employment, preferred.
  • Understanding of the impact of obtaining and maintaining employment for individuals with disabilities or barriers to employment.
  • Ability to communicate effectively, both verbal and written, with program participants and families, colleagues, and community partners.
  • Ability to work with others in an effective manner, earning their trust and confidence, enabling them to share their feelings to facilitate successful participation in programming.
  • Ability to clearly document program activities via case notes, written reports, and other written or electronic media.
  • Ability to perform computer functions, including the use of the internet, Microsoft Office, smart phones, and other productivity tools. Ability to learn agency client database management system.
  • Organizational and time management skills
  • Detail oriented, ability to gather and record data efficiently and accurately.

Basic Requirements:

EDUCATION: Bachelor's degree in education, social work, psychology, rehabilitation counseling, business, or related field preferred, or three years of experience providing case management services and High School Diploma or equivalent. 

CERTIFICATION:  Must have valid driver's license and automotive insurance. Must regularly provide the agency with proof of valid auto insurance. Certification in First Aid and CPR is required. 

WORK CONDITIONS: Works a flexible and varying schedule that may include nights or weekends as determined by the needs of the individual served. Ability to handle a variety of tasks that change often. Ability to work independently with moderate supervision. Travel is required within communities served and may include transporting the individual served as necessary. Must be able to perform physical requirements of the position, including, but not limited to inside, sedentary work. Frequent forward, horizontal reach at desk. Occasional standing, walking up to 60 feet, stair climbing, stooping, bending; push/pull up to 10 pounds. Infrequent crouching/squatting. Working conditions will vary according to each participants work site. Standing, lifting, stooping, twisting, and hand/arm manipulation may occur at any given job site.

EQUIPMENT: General office equipment, computer, phone systems, adaptive equipment. May utilize other equipment such as floor machine, pallet jack, tow motor, based on the needs of the individual served.
